Apply on recruiter's website Integration and Partnership Locality Lead Salary £38k to £46k Location Countywide Permanent full time contract (37 hours per week) Essex County Council Adult Social Care is moving towards a transformational shift, from a focus on crisis long-term care support to a focus on early intervention and prevention and more joined-up working with Health. Across Essex, we support over 10,000 vulnerable older adults, mainly over the age of 70, every year. We also support over 3,500 vulnerable adults of working age who have learning disabilities, nearly 700 adults with mental health needs and over 2,100 adults with physical impairments. We know that the earlier we can identify issues, the more likely it is we can support them to retain as much independence as possible and to live within their own homes. By doing more earlier on to Prevent, Reduce and Delay needs from growing, we can help support people to enjoy personal health and wellbeing. As Integration and Partnership Locality Lead, you will work with our operational teams and NHS and other partners to design and deliver the integration of health and social care. The role will support our four quadrant areas (each the size of a large London borough) in developing relationships with key partners, to identify best practice. A proven track record of influencing and the ability to build strong collaborative working relationships across the service to support operational delivery are essential in this role. Skills and Knowledge Educated to degree level or with equivalent professional experience. Evidence of continuing professional development and expert knowledge in social care. Experience of project management /delivery. An understanding gained through experience in the broader integration of health and social care and implications for delivery planning at local level. Experience of working with stakeholders and partners to agree and deliver appropriate commercial structures that engender delivery. Strong leadership and behavioural skills with the ability to focus on results, forging a highly performing and motivated team from diverse backgrounds to achieve strategic objectives. Strong influencing skills with an ability to proactively lead and deliver change within a complex multi-agency environment. Proven ability of developing solutions to deliver quality service to customers with financial constraints and competing priorities. Proven track record of using professional expertise to deliver objectives and outcomes while managing conflicting priorities.
